Escondido Post Acute Rehab has been meeting our community's needs for more than 50 years! Whether for a short stay to help you recover from a recent surgery or illness, or for the long-term care of a loved one, we're here for you!

Community Outreach

Escondido Post Acute Rehab provides post-acute skilled nursing and rehabilitation services for patients recovering from the effects of disabling injuries and illnesses, and long-term care for persons whose needs cannot be met in a lower level of care setting. Our goal is to provide a smooth transition from “hospital to home” whenever possible; or a comfortable, caring environment for those needing to make our Escondido Post Acute Rehab their new home.

It is our mission to provide high quality, post-acute skilled nursing and rehabilitation services for patients recovering from the effects of disabling injuries and illnesses, and long-term care for persons whose needs cannot be met in a lower level of care setting. Through the coordinated efforts of our multi-disciplinary staff, we seek to provide a smooth transition from “hospital to home” whenever possible; or a comfortable, caring environment for those needing to make our Escondido Post Acute Rehab their new home.

MEET OUR HEALING HEROES

Rob Zitsman, Administrator

Rob has more than 30 years' health care experience spanning the continuum of care from acute care hospitals to post acute, skilled nursing care. He has been a licensed skilled nursing facility administrator since 1997. Our staff, patients and their families benefit from his knowledge and experience of our complex healthcare system.

Rob is an Ohio native with an undergraduate degree in business from The Ohio State University and a master's degree in business from Wright State University.

Angelica Lorenzana, RN, BSN, Director of Nursing

Angeilica "Angie" has more than 20 years' experience as a registered nurse in skilled nursing and rehabilitation facilities. Over these years, she gained experience in all of the clinical aspects of skilled nursing and rehabilitation services. With this as a foundation, she chose to advance into the role of a Director of Nursing four years ago. She prides herself on being available to patients and families, to answer their questions and address their concerns. Angie is always seeking ways to better meet the needs and expectations of our patients, families, and our staff.
